Job Title: Case Manager
Position Number:
Work Days/Times: Monday-Friday;
Hours per Week: 40
Location: Palatine & Waukegan, IL
Description: The Case Manager is responsible for the coordination of service delivery to meet the needs of the individuals with regard to their abilities and personal choices. The Case Manager is the liaison between the family or other service providers and conducts all staffings, meetings in which goals are established for the upcoming year. •Coordinate the interdisciplinary team process for: assessment, program planning, goal development, implementation and regular review of progress for the individual served. •Conduct pre-staffing meetings with individuals/guardians, in regard to accomplishments, program delivery and determination of who will be at the next staffing and when the next staffing will be held. Such meetings are also held with all involved staff. A written record of these meetings will be provided and maintained in the individual’s case records. •Ensure individuals have supports and are linked with resources such as: The Department of Rehabilitative Services, transportation providers, Social Security, Medicaid, etc. •Conduct monthly reviews of individual’s status in relation to their plan; provide needed adjustments or supports. Document in the individual’s case record any status change or adjustments. •Recommend individual discharges as deemed appropriate and conduct the discharge process according to established guidelines. •Coordinate data collection for statistical reporting as required to document character of caseloads and to develop caseload analysis. •Assist persons served as needed with daily personal hygiene needs including use of the restroom, hand washing, dietary needs, accidents, seizures, etc. General Areas of Responsibility: Advisory and Resources •Serve as a resource about needs, progress, and problems related to the individual’s served. Hours and Work Year •7:30 a.m. - 4:00 p.m. •40 hours per week. •12 months per year. •Exempt Classification
Requirements: Education/Training and Experience Requirements •Bachelor’s Degree in the Human Service field. •Two years experience working within a social service agency with at least one-year experience devoted to service for persons with developmental disabilities. •Strong written and oral communications skills. •Possess a valid Illinois driver’s license and a car available for work-related transportation needs. •Prior experience related to the field of Case Management desirable. Physical Requirements •Able to engage in physical activities required for the implementation of Non-Violent Crisis Prevention techniques (CPI), CPR and First Aid.

Job Title: Training Specialist

Work Days/Times: 5 days a week; 7::30-4:00pm
Hours per Week: 40
Location: Palatine and Waukegan
Description: The Training Specialist is the individual within the facility who provides direct training delivery to individuals served utilizing contract work and/or nonwork activities. •Implement all objectives for persons served according to the individual's defined plan. •Maintain Protection of Rights for Persons Served as outlined by procedure. •Assist persons served as needed with daily personal needs including use of the restroom, hand washing, dietary needs, accidents, seizures, etc. •Implement behavior management techniques as required. •Maintain accurate, legible data on the performance of persons served including time sheets, performance on objectives, assessments, incident reports, accident - illness - seizure documentation, etc. Communicate data verbally and/or in written form in a timely manner as required by procedures. •Keep supervisor, or appropriate staff informed of all essential job functions. •Ensure that individuals perform contract jobs and participate in nonwork activities in accordance with procedures and as indicated in the service plan. •Provide constant supervision for those clients assigned and others as the need or duty requires.
Requirements: •High school degree or equivalent. College degree preferred. •Demonstrate good written and oral communication skills. •Previous exposure or experience with adults with disabilities desired. Able to lift 40 lbs, using appropriate lifting techniques. Able to engage in physical activities required for implementation of approved CPI, CPR and First Aid techniques.

Job Title: TRAINING SPECIALIST/AIDE

Hours per Week: 40
Location: PALATINE
Description: The Training Specialist/Aide is the individual within the facility who provides direct training delivery to individuals served utilizing contract work and/or nonwork activities. The Training Specialist/Aide assists individuals with personal care/hygiene issues, coverage for areas due to staff absences, extra duties for bus coverage duties, driving, lunch coverage with clients, and other assigned duties by supervisor. Assists persons served as needed with daily personal needs including assisting individuals with use of the restroom. Lifting individuals while assisting them with daily personal hygiene needs in the restroom, hand washing, dietary needs, accidents, seizures, etc. Implements behavior management techniques as required. Maintains accurate, legible data on the performance of persons served including time sheets, performance on objectives, assessments, incident reports, accident - illness - seizure documentation, etc. Ensures that individuals perform contract jobs and participate in nonwork activities in accordance with procedures and as indicated in the service plan. Provides constant supervision for those clients assigned and others as the need or duty requires. Keep supervisor, or appropriate staff informed of all essential job functions. Ensure that individuals perform contract jobs and participate in nonwork activities in accordance with procedures and as indicated in the service plan. Provide supervision during lunch periods, assuring proper sanitation and food preservation of consumer lunches and observe dietary guidelines as needed.
Requirements: EDUCATION/TRAINING AND EXPERIENCE REQUIREMENTS High school degree or equivalent. College degree preferred. Demonstrate good written and oral communication skills. Valid Illinois Driver’s license and good driving record. Previous exposure or experience with adults with disabilities desired. PHYSICAL REQUIREMENTS Use approved training methods to insure work quality, and work behaviors while meeting established standards for the job. Ability to lift 40 pounds. Able to engage in physical activities required for implementation of approved CPI, CPR and First Aid techniques.

Job Title: Van Driver
Position Number: CC 102
Work Days/Times: M-F, 6:30-9:00am and 2:00-4:00pm
Hours per Week: 20
Location: Palatine and Waukegan
Description: Provide door-to-door transportation to adults with a disability. Provide transportation from individual's residences in the morning to the Agency workcenter. Provide transportation from Agency workcenter in the afternoon back to individuals' residences. Responsible for the safe operation of the van at all times.
Requirements: Valid driver's license and good driving record. Must be able to pass physical and criminal background check.

Job Title: Respite Worker
Position Number: CC 103
Work Days/Times: Flexible Hours
Hours per Week: Part-Time
Location: Palatine
Description: Assist in the development, training, implementation and data-tracking of Respite Client's tasks. Provide supervision, care and structured activities in relation to the Respite Client's goals, needs and capabilities. This should include educational, recreational and socially appropriate activities.
